All program forms are in PDF format. If you experience difficulty
in opening and downloading any of them, you may need to update
your Adobe Reader. You can do so by clicking this link to

website and follow the download instructions. Once the
download is complete and you restart your computer, you should
be able to open the PDF files.
The completed
forms can be mailed to the Natick Comets (P.O. Box 382, Natick,
MA 01760) or hand delivered on the first day of your child's
registration at the William L. Chase Arena.
